The Seventh Vermont Blueprint for Health Annual Conference will provide an introduction to self-management support in primary care, discuss systems redesign to support self-management, and outline the financial viability of self-management support and group medical visits.   The Blueprint is guiding a comprehensive  and statewide process of health system transformation designed to reduce the health and economic impact of the most common chronic conditions and focus on their prevention.
